Search Results and Listings
Content using on DocStats primarily comes from the Centers for Medicare & Medicaid Services (CMS.gov) published data and the FOIA-disclosable NPPES health care provider data file. That is means:
The information disclosed on the National Provider Identifier (NPI) Registry and in the downloadable files are Freedom of Information Act (FOIA) disclosable and public information. This information is required to be disclosed under the FOIA and FOIA amendments. There is no way to "opt out" or "suppress" the NPPES record data for health care providers with an active NPI.
The National Plan and Provider Enumeration System (NPPES) Data Dissemination Notice and outreach documents from CMS encourage health care providers to review their NPPES data and update any information to keep their record current.
If the downloadable file or the NPI Registry reflects information that is incorrect, health care providers should correct their information by visiting https://nppes.cms.hhs.gov, or by obtaining a paper NPI Application/Update Form (CMS-10114) from the NPI Enumerator or from the CMS forms page (https://www.cms.gov/Medicare/CMS-Forms/CMS-Forms) and mailing the completed, signed form to the NPI Enumerator. Providers who need assistance in editing their records should contact the NPI Enumerator by phone at 1-800-465-3203, by email firstname.lastname@example.org, or by letter: NPI Enumerator, P.O. Box 6059, Fargo, ND 58108-6059.
Health care providers may also update their information online by visiting https://nppes.cms.hhs.gov/ and following the on-screen instructions.
Once the data has been updated by the provider, allow up to 6-8 weeks for changes to be reflected on DocStats.com. CMS.gov makes the information available to the public on a regular basis and does not instantly update the information for public use.

There are no conditions to the effectiveness of this Agreement. This Agreement contains the entire agreement and understanding of the parties hereto, and supersedes any prior agreements or understandings between or among the parties hereto, with respect to the subject matter hereof.
This Agreement and the rights of the parties hereto, shall be governed by, construed, and enforced in accordance with the laws of the Commonwealth of Pennsylvania. Any action in respect of, or concerning, this Agreement shall be litigated solely in Indiana County, Pennsylvania, and both parties consent to jurisdiction of the person and venue solely in Indiana County, Pennsylvania.
We may terminate your access to DocStats, without cause or notice, which may result in the forfeiture and destruction of all information associated with you. All provisions of this Agreement that by their nature should survive termination shall survive termination, including, without limitation, ownership provisions, warranty disclaimers, indemnity, and limitations of liability.
Changes To This Agreement
We reserve the right, at our sole discretion, to modify or replace these Terms and Conditions by posting the updated terms on DocStats. Your continued use of DocStats after any such changes constitutes your acceptance of the new Terms and Conditions.
Please review this Agreement periodically for changes. If you do not agree to any of this Agreement or any changes to this Agreement, do not use, access or continue to access DocStats or discontinue any use of DocStats immediately.
